Using our wealth of experience and expertise, we have also developed a broad range of programs that focus on specific conditions. Whether you are recovering from a hospitalization or illness, or living with a chronic disease like COPD or Diabetes, HarmonyCares Home Health can help. We are Medicare certified and accredited by the Community Health Accreditation Program (CHAP). Home Health services are covered 100% by Medicare and are provided at no direct cost to the patient.

Anodyne Therapy

Anodyne (Infrared Light) Therapy is a non-invasive way to increase circulation and reduce pain. State of the art equipment is used in the area affected in order to increase blood flow and reduce pain, stiffness, and muscle spasms caused by underlying health conditions. Anodyne is safe and effective for many conditions.

Congestive Heart Failure

Congestive Health Failure is one of the most common reasons for hospital admission and each patient’s disease progression varies. HarmonyCares Home Health program is designed to prevent recurrent repeated hospital visits through the use of physical therapy and nursing interventions that are based on your individual treatment goals We focus on diet, disease, and medication management.

COPD

Did you know that the Center for Disease Control (CDC) has rated Chronic Obstructive Pulmonary Disease (COPD) to be the third leading cause of death amongst Americans? Here at HarmonyCares Home Health, we realize the difficulties associated with living with COPD. Our skilled clinicians provide care, therapies and education that will assist in your ability to manage your COPD and decrease the amount of emergency room visits.

Dementia

Did you know that according to the World Health Organization a new case of Dementia is diagnosed every 4 seconds? Our dementia program focuses on the clinical symptoms of this debilitating disease that are often severe enough to interfere with daily functioning and quality of life.

Diabetes

Using the cornerstones of diabetes management (meal planning, exercise, medication, and blood glucose monitoring), our skilled clinicians will work to establish a plan of care that teaches new skills and educates family and caregivers on the ever-changing treatment of this disease. HarmonyCares Home Health utilizes the skills of its registered nurses and dietitians to develop and assist patients with realistic meal planning. Infrared light therapy is also utilized and geared toward improving functional mobility and reducing fall risk.

Fall Risk Reduction Program

Falls are one of the greatest risks to people age 60+. In fact, according to the National Center for Injury Prevention and Control, one in three adults age 65 and older falls each year. Our Optimum Balance program provides balance training to help reduce the need for emergency care due to falls. Our outcomes are proven: Patients who take part in the program are consistently able to reduce their fall risk as measured by standardized testing.

Pain Management

Tens of millions of Americans suffer from chronic pain. HarmonyCares Home Health can help to ease and prevent pain starting with a comprehensive assessment. Safe and effective pain management includes physical therapy, medication management, medical social services for counseling, and a home health aide to assist with activities of daily living due to decreased functional ability. Our care professionals will work with you to develop pain goals.

Senior Safety

One in three people over the age of 65 will suffer a fall, which is the leading cause of death and hospitalization for older adults. This program’s focus is improved balance through interventions from physical therapy as well as improved over all safety through nursing interventions. These include medication management and review drug reconciliation, prevention of dehydration which leads to low blood pressure orthostatic hypotension, and pain assessment and interventions. In addition, all patients on the program will receive a detailed home inspection for safety which includes simple preventative advice and recommendations for home medical equipment.

Wound Care Management

If you have a non-healing wound or are recovering from surgery our nurses can help to prevent complications and preserve function. A certified wound care nurse oversees the care of all patients in the wound care program. All wounds are photographed weekly and photos are sent to the primary care physician for review. Weekly conferences are held with an interdisciplinary team to ensure that the best possible healing interventions are taking place.

Novel Coronavirus Care

Our interdisciplinary team of professionals will work with you to keep your patients out of the hospital and on the road to recovery following a diagnosis or suspected diagnosis of the novel coronavirus.
