Our Mission is to promote exceptional patient outcomes through compassionate delivery of care. HarmonyCares Home Health strives to deliver innovative, dependable, and compassionate care to our delicate aging population in the comfort of the home environment. With our passion for healing and commitment to excellence, we seek to exceed the accepted standards of our industry. We strive to create an environment and support system that allows older adults to age in place.

HarmonyCares Home Health is pursuing designation as an Age-Friendly Health System through our accrediting organization CHAP (Community Health Accreditation Partner). By working towards this special accreditation, HarmonyCares demonstrates our dedication to providing high quality, age appropriate care. Age-Friendly Care is focused on the 4M’s these include: what Matters most to you, your family, and caregivers, safely managing your Medications: developing strategies to help you increase Mobility around your home safely to decrease falls, and injuries: focus on the Mind and monitoring cognitive functioning as well as watching for signs of depression with aging. HarmonyCares is dedicated to providing respectful care, which is based on your individual preferences, and engages our patients as active partners throughout the course of their care.

Skilled Nursing

Perform wound care, injections, lab draws, catheter changes, and teach and train the family on how to manage medications, prevent hospitalizations, and manage their disease process.

Physical Therapy

Provide balance and mobility training as well as gait and strength training.

Occupational Therapy

Provider assistance to improve the patient's ability to perform the activities of daily living and assist with adaptive equipment, home medical equipment, and home modifications.

Speech Therapy

Assist with speech clarity and swallowing exercises often impaired after a stroke. Provide cognitive interventions to dementia patients to slow disease progression.

Medical Social Work

Provide counseling services to patients and families, assist with community resources (such as provider services, Meals on Wheels, and support groups) and act as a patient advocate.
